By placing a request for Registration, you consent to the collection and use of personal information about you and the Child, namely the fact that you have submitted a request for Registration, together with the details of the Registration, for the purposes of reviewing the request for registration and completing the Registration as well as for GTHL`s, Hillcrest Sports and Recreation Group, Hillcrest Sports Group and the Hillcrest Canadiens internal accounting, business, marketing and operational purposes. Such personal information will be collected, stored, used, disclosed and retained as described in GTHL`s Online Privacy Policy. You acknowledge and understand that GTHL, Hillcrest Sports and Recreation Group, Hillcrest Sports Group and the Hillcrest Canadiens has implemented and will continue to implement recording equipment in various arenas where GTHL, Hillcrest Sports and Recreation Group, Hillcrest Sports Group and the Hillcrest Canadiens games are played (“Subject Arenas”) and that GTHL intends to record GTHL and Hillcrest Canadiens games that take place in the Subject Arenas (each, an “Event”) and that such video recordings are intended to be published, distributed and otherwise exploited for commercial purposes by Hillcrest Sports Group, Hillcrest Sports and Recreation Group, Hillcrest Canadiens, GTHL, Hockey Canada and the Ontario Hockey Federation (collectively, the “GTHL Group”). You hereby agree on your behalf, on behalf of any other parent guardian or legal guardian and on behalf of the Child, to grant the GTHL Group, Hillcrest Sports and Recreation Group, Hillcrest Sports Group, Hillcrest Canadiens and all licensees, service providers, successors, legal representatives and assigns of the GTHL Group, the absolute and irrevocable right and permission to record the Child in the Subject Arenas, to capture and use his or her voice, image, appearance, likeness and name, and to use, reproduce, edit, modify, distort, adapt, translate, dub, condense, segment, editorialize, create derivative works, exhibit, project, display, publish, broadcast, distribute (including, without limitation, on social media sites, video-sharing websites, content aggregation websites, subscription websites and other internet portals, platforms and websites, by way of webcast, live or on demand video streaming, as well as in regard to all other forms of media and on any other medium, now in existence or which may later come into existence), import, rent, license, sell, commercialize or otherwise exploit throughout the world (collectively, “Use”) photographic pictures, moving pictures, audio-visual recordings and/or video-recorded images of the Child created at or in conjunction with the Event with or without his or her voice, or in which he or she may be included in whole or in part, and any of his or her possessions (collectively, “Recordings”) and, thereafter, to Use the same in all forms and media and on any medium, whether now existing or which may later come into existence, for art, advertising, trade, commercialization, competition of every description and/or any other lawful purpose whatsoever. You hereby acknowledge and agree, on your behalf, on behalf of any other parent guardian or legal guardian and on behalf of the Child, that none of you will have any right, title or interest in any pictures, images or recordings or in any forms or media thereof produced by or for the GTHL, Hillcrest Sports and Recreation Group, Hillcrest Sports Group, Hillcrest Canadiens nor will any of you be entitled to any form of remuneration whatsoever with respect to creation of any of the foregoing or any Use of any of the foregoing. You hereby waive, on your own behalf, on behalf of any other parent guardian or other legal guardian and on behalf of the Child, any right that any of you may have to inspect and/or approve the Recordings and/or finished product, any modified version and/or any derivative work thereof and you hereby waive, on your own behalf, on behalf of any other parent guardian or other legal guardian and on behalf of the Child, any right that any of you may have to control the Use of any of the foregoing. You hereby represent and warrant that any other parent guardian or legal guardian has expressly agreed to all of the foregoing. You personally release and forever discharge, on your own behalf, on behalf of any other parent guardian or legal guardian and on behalf of the Child, the GTHL Group, Hillcrest Sports and Recreation Group, Hillcrest Sports Group, Hillcrest Canadiens their service providers, licensees, successors, legal representatives, agents and assigns from any liability by virtue of any Use of the Recordings and from liability for violation of any personal or proprietary right that the Child may have in connection with the making of the Recordings and with any Use thereof. You shall indemnify the GTHL Group, Hillcrest Sports and Recreation Group, Hillcrest Sports Group, Hillcrest Canadiens their service providers, licensees, successors, legal representatives, agents and assigns with respect to any damages, costs, expenses, liabilities, fines, penalties, or other losses suffered by any of them in regard to any challenge by such child or other parent guardian or legal guardian in regard to the rights conferred to the GTHL Group Hillcrest Sports and Recreation Group, Hillcrest Sports Group, Hillcrest Canadiens hereunder and/or the exercise thereof by the GTHL Group
